Why Does Your Stomach Bloat Every Evening? Could It Be a 'SIBO' Signal From Your Small Intestine?

You've probably experienced it: your clothes fit perfectly in the morning, but by the afternoon, your abdomen becomes so bloated that you secretly unzipped your pants.

You've tried taking digestive aids and diligently consumed probiotics that are said to be good.

But it's only temporary; by the next afternoon, your belly is inevitably full of gas, and the uncomfortable bloated feeling saps even your daily vitality. Why do your efforts repeatedly fail? Could it be that all your efforts were chasing the 'wrong culprit' from the start?

Hello. I am Choi Yeon-seung, a Korean medicine doctor who has treated patients suffering from unexplained digestive diseases for 15 years, and I have been uncovering the connection between 'Damjeok (痰積)' and 'gut bacteria' as the fundamental cause of abdominal bloating.

If you read this post to the end today, you will clearly understand why your belly felt uncomfortable specifically in the afternoon and uncover the secret behind that persistent 'time-lag attack'.

And finally, I promise that you will hold the first key to breaking this uncomfortable cycle.

Morning vs. Afternoon: The 'Time-Lag War' Happening in My Belly

Morning and afternoon, why does my belly have two different faces?

To understand that secret, let's follow the 'long journey of food' that occurs within our bodies.

Morning (Fasting State): 'A Quiet and Clean House'

Our intestines, after a long night of fasting, are like a 'quiet and cleanly emptied house.' With no guests (food) to throw a party inside, the mischievous bacteria are also quietly biding their time. That's why in the morning, your belly can remain flat and comfortable.

Afternoon (After Meals): 'A Boisterous Party of Uninvited Guests'

However, the moment you have lunch, 'uninvited guests' rush into this quiet house, and a boisterous party begins. These guests are none other than bacteria that have excessively proliferated in the small intestine, where nutrients should be absorbed.

Normally, the carbohydrates, proteins, and fats we consume should be cleanly digested and absorbed in the small intestine.

However, if there are too many bacteria in the small intestine, these bacteria intercept the nutrients first and begin to ferment them as they please. As a result, a tremendous amount of 'gas' is produced.

It typically takes 2 to 3 hours for the food we consume to pass through the stomach and reach the small intestine.

From this moment on, the bacteria's 'gas-producing party' truly begins. This is the secret behind the 'time-lag attack' where your belly starts to gurgle and bloat around three or four in the afternoon, after lunch.

In the medical community, this condition, where bacteria abnormally overgrow in the small intestine causing various issues such as gas, abdominal bloating, diarrhea, and constipation,

is called 'SIBO (Small Intestinal Bacterial Overgrowth)'.

Ultimately, the 'time-lag attack' where your belly is fine in the morning but bloated by the afternoon, was not because you are overly sensitive, but rather a very characteristic 'distress signal' sent by the bacteria in your small intestine.

Why Did My Small Intestine Become a 'Gas Party Zone'? (feat. Damjeok)

Then the next question arises.

Why did *my* small intestine specifically become a 'gas party zone' for bacteria? Why aren't nutrients absorbed in time, instead becoming food for bacteria?

The fundamental cause lies in 'reduced small intestinal motility'.

A healthy small intestine propels food downward through peristaltic movements, and between meals, it thoroughly cleanses out residue with powerful contractions known as the 'Migrating Motor Complex (MMC)', much like cleaning the intestines.

In other words, it doesn't give bacteria a chance to throw a party, nor does it allow leftover waste from a party to accumulate.

However, if small intestinal motility significantly decreases for various reasons, food remains in the intestines longer, and intestinal cleansing is not properly carried out. For bacteria, this literally creates the optimal environment to throw a party.

And in Korean medicine, this state where intestinal movement slows down, and undigested food and waste accumulate, becoming damp and hardened, has been given a special name: 'Damjeok (痰積)'.

To put Damjeok simply in a metaphor, it is like a 'damp and hardened soil' where 'weeds' (bacteria) are bound to grow luxuriantly.

Here's a common mistake many people make: they diligently take only 'probiotics'.

But think about it. If you sow good seeds (probiotics) on overgrown, hardened soil full of weeds, will they sprout well? More often than not, they will either become food for the weeds or fail to grow properly.

Therefore, the fundamental solution to SIBO and abdominal bloating must begin not merely by killing bacteria or introducing probiotics, but by first improving the 'soil environment' of Damjeok.

What is Your Gas Type? Hydrogen vs. Methane (Mini Self-Diagnosis)

Surprisingly, the gas that bloats your belly is not just one type. The type of gas produced varies depending on which bacteria are dominant, and symptoms subtly differ accordingly.

Just as people have different personalities, bacteria also have their own distinct characteristics. I will introduce two representative types, so check which one you relate to more closely.

1. Hydrogen (H₂) Dominant Type: The 'Gurgling Diarrhea' Type

  • Characteristics: This occurs when bacteria that primarily produce hydrogen gas proliferate. This hydrogen gas irritates the intestines and increases intestinal water content, making bowel movements abnormally fast.
  • Typical Symptoms: You tend to feel a gurgling sensation in your abdomen and experience frequent diarrhea or loose stools.

2. Methane (CH₄) Dominant Type: The 'Severely Constipated' Type

  • Characteristics: This occurs when 'methanogens', which produce methane gas, excessively proliferate. It is also precisely called IMO (Intestinal Methanogen Overgrowth). Methane gas itself acts on intestinal nerves, significantly slowing down peristaltic movements.
  • Typical Symptoms: You often experience a belly so bloated it feels like it might burst from gas, along with severe constipation where you cannot have a bowel movement for several days, and a general feeling of fullness.

What's Your Gas Type? (Mini Self-Diagnosis)

□ My belly gurgles, and I go to the bathroom frequently. (Diarrhea tendency) ➜ Possible Hydrogen Type

□ My belly is bloated, but I haven't been able to go to the bathroom for days. (Constipation tendency) ➜ Possible Methane Type

Identifying the type of gas in this way becomes a very important clue in setting the direction of treatment. This is because the Korean herbal medicines or treatment strategies used vary depending on the type of causative bacteria.

The First Key to Escaping the Gas Chamber

Now you know the real reason why your belly was bloated every evening, right?

It wasn't simply because you were sensitive or had slightly weak digestive function. It was your small intestine, transformed into the damp and hardened 'soil' of Damjeok, and the excessively proliferated bacteria there, producing 'gas' and sending an earnest 'distress signal'.

Don't be frustrated by unexplained discomfort any longer. Your body wasn't wrong; it was sending us precise signals. Today, we have gained the first key to interpreting those signals.

But the story doesn't end here.

Today, we focused on 'why' gas accumulates, but a more important question remains.

'What' makes our intestines immobile and creates the pathological environment known as 'Damjeok'?

At its root are two significant causes often overlooked by many: 'low stomach acid' and 'stress'.

Next time, on the topic of <The Betrayal of Digestive Aids: What Happens When Stomach Acid is Insufficient>, we will delve into the second piece of this secret.

Until the day you reclaim your comfortable evenings, we will be with you.
