Causes and Symptoms
The Attack of 'Toxic Smoke,' Not 'Flames'
If typical gastroesophageal reflux disease (GERD) is like 'flames' burning the chest, reflux laryngitis is like 'toxic smoke' that silently rises and damages the throat.
Powerful stomach acid and digestive enzymes reflux beyond the esophagus, much higher, reaching the delicate 'larynx and vocal cords', which have no protective barrier.
Because the larynx and vocal cords have almost no defense against stomach acid, even a very small amount of reflux can easily cause inflammation and swelling.
This is why, despite no heartburn, the voice becomes hoarse (hoarseness), a foreign body sensation is felt in the throat (globus sensation), and chronic coughing persists.

The Stomach's Energy: Lost and Going in Reverse
In Korean medicine, the natural flow (順理, *sunri*) of the body's energy is considered to be from top to bottom.
Reflux laryngitis occurs when this natural flow is disrupted, and the stomach's qi (胃氣, *wegi*), which should descend, loses its way and 'goes in reverse', attacking the 'pharynx and larynx' (咽喉, *inhu*), the gateway to the respiratory system.
The biggest cause of this 'reverse flow' is 'traffic congestion' (肝氣鬱結, *gangi-ulgyeol*, Liver Qi Stagnation), where the flow of qi is blocked due to stress.
Uncirculated qi pushes the stomach's energy upwards in reverse.
Therefore, Korean medicine treatment goes beyond simply calming inflammation in the throat, focusing on resolving the root cause of 'traffic congestion' to normalize the flow of energy.
By dispersing congested qi due to stress (疏肝理氣, *sogan-igi*, soothing the Liver and regulating Qi), the pressure of reflux is reduced, and the weakened stomach qi is guided downwards, thereby restoring the body's order so that the path is no longer blocked or goes in reverse.

It is important not to further irritate the sensitive larynx and vocal cords and to give them time to recover.
- Post-Meal Habits
Lying down immediately after eating is the most dangerous habit that causes reflux. For at least 2-3 hours, sit or stand to allow enough time for food to descend in the stomach. - Diet Management
Spicy, greasy, and acidic foods, as well as coffee, alcohol, and carbonated drinks, promote stomach acid secretion and directly irritate the vocal cords. Eat bland and soft foods. - Voice Rest
Speaking loudly or repeatedly clearing your throat is like adding physical damage to already weakened vocal cords. Conserve your voice and drink water frequently to keep your throat moist.

Hoarseness and persistent throat clearing are often dismissed as simple fatigue and left untreated.
