Warts, why do they keep appearing only in this area?

Hello everyone. Today, let's talk about the site-specific characteristics of warts, a common condition that becomes surprisingly complex the more you learn about it. Warts can appear anywhere, but they tend to occur frequently and repeatedly in specific areas. And their shape and texture aren't solely explained by the type of virus.

1. Each Wart Virus Has a 'Preferred Site'

There are over 200 types of HPV that cause warts, and each virus prefers a specific skin environment. It's similar to how certain plants thrive in different conditions based on sunlight, shade, and humidity.

2. But Skin Also Differs by Body Site

The skin's structure and characteristics vary significantly by body site. Thickness, stratum corneum, sebaceous glands, sweat glands, degree of friction, and even the density of immune cells all differ. These differences mean that even the same HPV can produce completely different wart forms depending on the body site.

3. Why Warts Appear Differently by Body Site

👉 Finger Joints, Back of the Hand

These areas experience a lot of friction. Warts that form here are typically dome-shaped, firm, raised, and have a rough surface, sometimes with punctate bleeding in the center. Over time, keratin accumulates, transforming the lesion into a rough, grayish epidermis with deep infiltration that regrows even after shaving.

→ Treatment is generally effective, but autoinoculation is common. They can spread to other fingers or even the face after scratching or touching.
👉 Next to Fingernails, Nail Bed

Periungual warts frequently develop here. Nail-biting habits or injuries from nail removal can open a pathway for infection. The problem is that these are difficult to treat. Warts can grow under the nail, impeding nail growth and causing deformation or dystrophy of the nail plate itself. Laser and cryotherapy treatments are also challenging. Recurrence is common.
👉 Sole of the Foot

These are very well-known. Warts caused by HPV type 1 do not protrude above the surface; instead, they have a deeply embedded structure. Patients often mistake them for calluses, but a closer look reveals small blood vessels appearing as punctate bleeding in the center. These warts are typically deep, hard, and very painful. They cause severe pain due to pressure, and can even alter gait, affecting the knees. Treatment is also challenging, and recurrence is frequent as they often hide in deeper tissues.
👉 Face, especially Forehead or Cheeks

These are thin areas with active sebum secretion. Flat warts that appear here are small, skin-colored or light pink, and present as barely perceptible, thin papules. They typically appear not as one or two, but spread in a linear fashion, and are disseminated via autoinoculation through razors, fingernails, or cosmetic brushes. Since cosmetic concerns are particularly significant in these areas, treatment should be approached with minimal irritation and a focus on minimizing post-inflammatory hyperpigmentation.
👉 Eyelids, Around the Eyes

These are thin and sensitive skin areas. Multiple millet-sized flat warts appear here, sometimes spreading between the eyelashes or near the tear troughs. During treatment, various risks must be considered, including the potential for irritation-induced conjunctivitis, obstruction of vision, and scarring.
👉 Neck, Armpits, Chest

These are frequently scratched areas where skin folds and moisture are common. Multiple small, soft, papillary projections appear here, usually skin-colored and often connected in a line or clustered. In the early stages, differentiation from skin tags can be difficult. These areas are prone to spreading due to irritation, and autoinoculation leading to enlargement is common from scratching caused by clothing friction or necklaces.
👉 Palms & Between Toes

These are areas with thick skin and high sweat production. Mosaic warts are frequently observed here. This means multiple small warts coalesce to form a larger, plaque-like lesion. The surface is rough and thick, often appearing depressed in the center, and the surrounding skin is often hyperkeratotic with indistinct borders. Treatment, in particular, needs to penetrate deeply.

4. Warts are Less About a 'Small Virus' and More About the 'Larger Skin Environment'

While warts are a phenomenon caused by a small virus called HPV, their form, texture, depth, and recurrence reflect our skin environment and immune status. Is the skin thin or thick? Does it sweat a lot? Is there significant friction? Is the immune response active? All these factors explain "why that particular wart is there, in that specific shape, and remains so persistently." Therefore, wart treatment isn't simply about burning or removing them; true resolution comes from understanding and addressing the local environment and immune system of the affected area.
