Gout: Symptoms, Causes, and Treatment

Gouty arthritis is one of the primary reasons people seek medical attention, often more so than for general musculoskeletal pain, due to gout attacks accompanied by excruciating pain.
Causes and Mechanisms of Gout
Gout occurs when uric acid (uric acid), a purine metabolite, is not properly excreted. This leads to crystal formation, primarily in the joints, causing inflammation and manifesting as excruciating pain. The onset of this severe pain due to gout is termed a gout attack (gout attack), and these attacks often subside only to recur.
Inflammatory Pathway
Observing the inflammatory pathway in gout, when uric acid crystals, known as MSU crystals, stimulate macrophages, the IL-1 pathway is activated, triggering an inflammatory response. This response can be heightened by non-crystallized uric acid, which means acute gout attacks can occur even when crystals are not visible in a hyperuricemic state.
Autoinflammatory Diseases and Immune Response
Recently, the term "autoinflammatory disease" has been used in contrast to autoimmune conditions, drawing attention to the mechanisms of gout. It is hoped that a better understanding of immunity and inflammation will lead to more effective treatment applications.
Gout Treatment Guidelines
The following medications are used in the treatment of gout:
- NSAIDs: The most commonly used anti-inflammatory pain relievers
- Colchicine: Used as an inflammasome inhibitor
- Glucocorticoids: Utilized as steroids
Traditional Korean Medicine and Alternative Therapies
In Traditional Korean Medicine clinics, treatment is approached from the perspective of "expelling wind, dampness, and heat" (거풍습열), addressing the concept of "damp-heat in the joints." The focus is on controlling pain and restoring inflammatory responses through treatments such as acupuncture and herbal medicine, and strategies to facilitate smooth bowel and urinary excretion to prevent uric acid accumulation are also included.
