Restless Legs Syndrome, Uncomfortable Leg Sensations

"Why do my legs feel strange only at night?"

A woman in her late 50s has been experiencing an unpleasant sensation in her legs every night for the past 3-4 years, making it difficult for her to sleep. This sensation is not pain, but is described as a crawling, tightening, or burning feeling.

The symptoms worsen particularly when she lies down, and while moving her legs provides temporary relief, the sensation returns when she lies down again, preventing deep sleep.

Two years ago, she was diagnosed with Restless Legs Syndrome (RLS) and has been taking dopamine-agonist medications, but her symptoms are progressively worsening. Due to poor sleep, she also experiences chronic fatigue, depression, and heart palpitations.

Restless Legs Syndrome: What's the Underlying Issue?

  • Iron Supplements: If iron deficiency is confirmed, iron preparations are co-administered to improve neurotransmission.
  • Benzodiazepines (Sedatives)
    • Clonazepam (brand name: Rivotril): Calms the nerves to reduce symptoms and induce sleep.
  • Neuropathic Pain Medications
    • Gabapentin (brand name: Neurontin): Alleviates nerve hypersensitivity to reduce symptoms.

2. Problems with Pharmacological Treatment

  • Augmentation: Dopamine-agonist medications can lead to augmentation over time, where their effectiveness diminishes or symptoms worsen.
  • Side Effects: Long-term use can cause side effects such as nausea, dizziness, and palpitations.
  • While medications can temporarily alleviate symptoms, they may not always eliminate them completely.

    • Key acupoints: Shenmen (HT7), Xinshu (BL15), Pishu (BL20).

3. Lifestyle Management

  • Warm Therapy: Keep legs warm before bed to promote blood circulation.
  • Iron Intake: Consume iron-rich foods and, if necessary, take iron supplements.
  • Stress Management: Restore psychological stability through meditation, yoga, etc.
  • Regular Exercise: Engage in light aerobic exercise to relax leg muscles.
