A Burning Sensation – 5 Hidden Symptoms

1. A Burning Sensation in the Stomach – How True Is That Statement?

One of the phrases I often hear in the clinic is this:

“Doctor, my stomach is burning.”

Some say, "It feels like there's a fire inside." Others describe it as, "My stomach is sizzling, and it rises up to my chest."

When we hear this, it's easy to assume it's simply due to excessive stomach acid. But in reality, that might not be the case. The single phrase "my stomach is burning" can hide five or more entirely different pathological conditions. Today, I'll explain these five conditions one by one, from a symptomatic, pathological, and therapeutic perspective.

2. Case ① Acid Reflux Type – Burning Sensation Rising from the Stomach to the Chest

This is the most common case. It's a condition where stomach acid flows back into the esophagus after meals, causing a fiery burning sensation that rises to the chest.

Characteristics include:

  • Worsens when lying down or bending over after meals.
  • Accompanied by acid regurgitation, a sensation of phlegm in the throat, and a foreign body sensation in the throat.
  • Clear upward directionality.

This is due to a weakened lower esophageal sphincter (LES), the valve between the stomach and esophagus. Acid is fine within the stomach, but once it enters the esophagus, it becomes a strong painful irritant. Not only is acid suppression needed, but comprehensive treatment involving post-meal posture adjustment, weight loss, and esophageal mucosal protection is also required.

3. Case ② Mucosal Damage Gastritis – Epigastric Burning, Gnawing Pain, and Tenderness Upon Pressure

This is a case where the acid causes problems even when it remains within the stomach. In other words, stomach acid levels are normal, but the stomach lining has become weakened. Symptoms worsen on an empty stomach, slightly improve after eating, but soon return with a burning, gnawing sensation.

“My stomach feels sore and hot.”
“It’s more uncomfortable if I don’t eat anything.”

If you use such expressions, it means the protective barrier of your gastrointestinal mucosa has broken down. Treatment should focus on coating and regenerating the mucosa, rather than solely on acid suppressants.

4. Case ③ Autonomic Hypersensitivity Type – Normal Test Results, Yet the Brain's Perceived Fire

The third case is when there's a strong burning sensation in the stomach, despite endoscopy results being normal. This is a state of autonomic hypersensitivity related to functional gastrointestinal disorders or brain-gut axis dysfunction.

“My stomach feels like it’s boiling.”
“My endoscopy is normal, but I feel like I’m going crazy.”

When such expressions are used, it's not a problem with the stomach itself, but rather with the "brain interpreting the sensations." Stress, insomnia, tension, chronic emotional suppression, etc., are linked to the vagus nerve that controls the stomach, causing sensations more severe than actual inflammation, even when no inflammation is present. Treatment to stabilize the autonomic nervous system and reduce sensory hypersensitivity is key. Common acid suppressants can actually worsen the condition.

5. Case ④ Diabetic Gastroparesis – Food Isn't Passing Through, Yet the Stomach Burns

This is a somewhat unfamiliar but important pathological condition. It's a case where diabetes has paralyzed the stomach. When diabetes is prolonged, not only microvessels but also autonomic nerves, including the vagus nerve, are damaged. As a result, food doesn't empty from the stomach and remains stagnant for a long time, fermenting within the stomach and allowing acid to linger.

“It’s been 4 hours since I ate, but my stomach still feels full.”
“I feel bloated, but my stomach has a fiery, burning sensation.”

This is a problem with gastric emptying function. Even if acid doesn't reflux, it becomes toxic if it remains for a long time. Along with blood sugar management, medications that aid gastric emptying should be used.

6. Case ⑤ Traditional Korean Medicine Internal Heat Type – A State Where Heat Rises Upward, Causing a Burning Sensation in the Stomach and Chest Tightness

This is a state where the burning sensation is felt not due to the stomach itself, but due to disrupted energy (Qi) flow and emotional stagnation in the body. Characteristics are clear:

  • Burning sensation in the chest.
  • Dry mouth and red tip of the tongue.
  • Face easily flushes and palpitations occur.
  • Accompanied by insomnia, restlessness, and headaches.

This is less a problem of the stomach and more a situation where the flow of Qi and Blood and the distribution of heat in the body are disrupted. In Traditional Korean Medicine, this is classified under patterns like Liver Qi stagnation transforming into Heat, Stomach Heat, and Heart Heat ascending. In this case, suppressing acid is the wrong approach; instead, the core of treatment involves resolving stagnation, reducing heat/anger, and restoring the harmony of the Zang-fu organs.

7. A Burning Sensation in the Stomach – One Phrase, Five Pathologies

We all express symptoms with words. However, those words never directly point to the underlying disease.

“My stomach is burning.”

Within this phrase, there are:

  • Acid reflux
  • Mucosal damage
  • Sensory hypersensitivity
  • Gastric paralysis
  • Upward surging heat

Five completely different pathways are hidden. Therefore, the treatment must also be different. One type of acid suppressant cannot solve everything. True treatment begins by listening to the nuances of the words, dissecting the sensation, and understanding how the body is truly suffering. That single phrase—"my stomach is burning"—is ultimately a very precise signal sent by your body. Depending on how it's interpreted, the illness manifests completely differently.
