This period, where fasting blood sugar levels fluctuate between 100–125 mg/dL, acts as our body's final warning light.

In the past, I also struggled by trying to lose weight through mindless fasting, only to end up feeling lethargic with even more volatile blood sugar swings. Reflecting on that experience, I realized that the key is not simply reducing the number on the scale, but 'how' you restore the body's metabolism. Today, we will look closely at why weight management is essential during the prediabetic stage and how Traditional Korean Medicine (TKM) approaches this issue.

Why is a 5% Weight Loss the Golden Window for Prediabetes?

Receiving a prediabetes diagnosis can feel overwhelming, but this period is actually a precious opportunity to reverse the metabolic flow. According to research, losing just 5–10% of your initial body weight can reduce the risk of progressing to type 2 diabetes by more than 50%. This isn't just because fat is lost, but because your body's insulin resistance improves.

Insulin acts as a 'key' that lets glucose from the blood into the cells. However, when abdominal fat accumulates, this keyhole becomes 'rusty' and stops working properly. This is insulin resistance. By reducing visceral fat through weight management, you essentially clean the rusty keyhole, allowing insulin to function again and reducing the burden on the pancreas. When the function of the Spleen and Stomach (脾胃, Biwi) weakens, the food consumed cannot be converted into energy and instead turns into sticky waste called Phlegm-fluid (痰飮), which accumulates throughout the body. This Phlegm-fluid is a primary culprit that hinders blood flow and interferes with insulin action. Therefore, when managing weight for prediabetes, it is not about starving yourself; instead, you must strengthen the Spleen and Stomach functions to discharge waste and assist metabolic circulation. Once the body is 'cleansed,' we often observe blood sugar levels stabilizing naturally.

The Mechanism of 'False Hunger' During Prediabetic Weight Management

The biggest obstacle when starting weight loss is 'uncontrollable appetite.' Because those in the prediabetic stage experience significant blood sugar fluctuations, the brain declares an emergency when blood sugar drops sharply, triggering intense hunger. This is often called 'false hunger.' If you reach for sweets at this moment, you fall into a vicious cycle where blood sugar spikes and crashes again.

When this pattern repeats, it becomes difficult to endure through willpower alone. The brain constantly sends signals that energy is lacking, even though the body is paradoxically overflowing with fat. In such cases, it is important to maintain regular meal intervals and compose a fiber-oriented diet to prevent blood sugar spikes. Furthermore, in TKM, prescriptions that lower Heart-fire (心火, Simhwa) to prevent stress-induced binge eating and cool the heat in the stomach to manage abnormal hunger can be very helpful.

Activating Fat Metabolism with Bangpungtongseong-san (防風通聖散) and Metabolic Management

One of the prescriptions frequently used in TKM to assist with prediabetic weight management is Bangpungtongseong-san (防風通聖散). Beyond simply suppressing appetite, this formula works to discharge toxins and waste accumulated in the body through urine, stool, and sweat. It can offer significant effects, particularly for those with severe abdominal obesity and a tendency toward constipation.

Bangpungtongseong-san (防風通聖散) has a mechanism that increases the basal metabolic rate and aids in fat oxidation, making it effective for turning on the metabolic switch that has stalled in the prediabetic stage. Of course, this is not applied uniformly to everyone, but it serves as an excellent alternative for those with high internal heat and stagnant waste. At the clinic, we recommend herbal medicines delicately adjusted based on this basic prescription, considering the patient's current blood sugar status and level of fatigue.

What if Weight Drops but Blood Sugar Stays the Same?

Occasionally, patients are disappointed when they lose 3–4kg but their blood sugar levels remain unchanged. In this case, two things must be checked: changes in muscle mass, and sleep/stress levels. Muscle is like a 'vacuum cleaner' that consumes the most glucose in our body. If you lost only muscle through improper fasting, your blood sugar control capacity may actually decrease despite the weight loss.

Additionally, if you lack sleep or suffer from chronic stress, the hormone cortisol is secreted. This hormone interferes with insulin action and causes the liver to continuously produce glucose. In other words, the body stays in 'battle mode' and tries to keep blood sugar high. Therefore, prediabetic weight management must be supported by maintaining muscle mass and getting sufficient rest for the numbers on the scale to translate into true clinical improvement.

Q. Is it possible to escape prediabetes through diet control alone?

Diet is the most fundamental element. However, if insulin resistance is already high, the rate of metabolic recovery through diet alone may be slow.
