Why Facial Hives Appear | Songdo Hives

Hello, this is Baengnokdam Korean Medicine Clinic.
Hives that keep appearing on your face, could it be just a simple allergy?
Someone who came to our clinic said this:
“These days, something keeps appearing around my eyes and on my cheeks every morning. I don't even want to take off my mask, let alone put on makeup... I've become too sensitive.”
Indeed, hives themselves are a common symptom that many people experience. However, when they start appearing repeatedly on the face, the situation changes a bit. This is because the skin on the face is immediately visible to others. Especially when symptoms repeatedly occur in areas connected to emotions, such as around the eyes, cheeks, and mouth, it should be viewed not just as a simple skin problem, but as a warning signal from the entire body.
General Mechanism of Facial Hives
Firstly, ordinary hives are usually caused by a histamine reaction. The immune system overreacts to certain stimuli—such as food, environment, temperature changes, emotional stress, etc.—leading to vasodilation, and the appearance of red patches or itchiness on the skin.
However, the problem is that the face is particularly more sensitive. It has a rich blood supply and a complex distribution of sensory and autonomic nerves, so even a slight accumulation of stress or fatigue can immediately cause it to redden, itch, or feel hot.
Why medication doesn't work – 'Histamine Syndrome'?
Many patients report that even after taking antihistamines, the effect is only temporary, and the symptoms recur after a few days. In such cases, it's not about 'reducing histamine', but rather about addressing the 'body's underlying state that continuously produces histamine'.
Especially these days, the term 'Histamine Intolerance' is frequently used. This refers to a condition where the body lacks the enzyme (DAO) that breaks down histamine, leading to a continuous buildup of histamine even with slight stimulation, causing the body to overreact. It's not just a simple immune hypersensitivity, but a problem of metabolic and constitutional sensitivity.
How does Traditional Korean Medicine view facial hives?
In Traditional Korean Medicine, recurrent facial hives are interpreted as issues of internal heat, stagnation, and circulation problems. For example, for those experiencing a lot of stress, it's seen as Liver Qi Stagnation (간기울결). For those with excessive heat in the intestines, it's Spleen-Stomach Damp-Heat (비위습열). If the face frequently reddens and feels hot, it's often attributed to Heart Fire Rising (심열상염). For those with a cold and weak body, it's considered a Deficiency of Essence and Blood (영혈허약) or insufficient Defensive Qi (기표불고).
In other words, even though the 'hives' appear to be the same outcome, the underlying pathological conditions are entirely different, and thus the treatment approach must also differ.
Symptoms to look for beyond just hives
There are also other signs frequently found in individuals experiencing such facial symptoms.
- Abdominal bloating, indigestion, frequent belching or flatulence
- Frequent sensations of heat and easy facial flushing
- Palpitations, insomnia, severe mood swings
- Loose stools, alternating constipation and diarrhea
These systemic symptoms are ultimately linked to issues with organ function, nervous system regulation, and Qi-blood circulation. Therefore, it's not just about treating facial hives, but about stabilizing and regulating the entire body.
Treatment Approach – Treating the body's flow, not just the skin
In herbal medicine treatment, prescriptions are typically tailored to the individual's constitution to reduce stagnant heat and calm the upward-flowing Qi to the skin surface. For example, if Liver Qi Stagnation is predominant → Bupleurum (시호), Cyperus (향부자), Gardenia (치자) are used. If Spleen-Stomach Damp-Heat is central → Atractylodes (창출), Coptis (황련), White Atractylodes (백출), Poria (복령) are used. In cases of overlapping heat syndrome and blood heat → Rehmannia (생지황), Moutan (목단피), Red Peony (적작약) are used. For individuals with Qi deficiency → tonic herbs like Astragalus (황기), Angelica Gigas (당귀), Ginseng (인삼) are administered.
Acupuncture treatment primarily focuses on the Liver, Gallbladder, Large Intestine, and Lung meridians to reduce heat or promote circulation. Acupuncture stimulation on the facial area is used only when strictly necessary, with an approach aimed at stabilizing the overall meridian flow.
Signals from the Skin: Emotions and Internal Organs
Symptoms appearing on the face fully reflect a person's emotions, fatigue, and hypersensitivity. Do not dismiss frequently recurring facial hives as mere skin troubles or allergies; rather, acknowledge them as small signals sent by your entire body. Instead of reapplying makeup or continuously using medication, it is now time for a fundamental recalibration.
