Fibromyalgia Examination: Which tests are necessary and why?

There is no single blood test or imaging scan that can definitively diagnose fibromyalgia. Diagnosis is based on confirming the extent of pain, fatigue, sleep, and cognitive symptoms, as well as physical examination findings. Depending on the medical history and examination, specific tests are selected to identify other conditions that may cause similar symptoms. A normal test result does not mean that pain is not present.

Tests and Diagnosis

Fibromyalgia: Illustration for this condition

Key points

  1. 1Diagnosis is not made with a single specific test.
  2. 2Tests are selected based on medical history and examination to identify other potential causes.
  3. 3The core of the diagnosis is the clinical evaluation of symptoms and daily functioning.

Test Selection Support

What tests are necessary?

There is no single test that can immediately confirm a diagnosis of fibromyalgia. First, we look at symptom patterns through medical history and physical examination, and based on those results, we choose tests to identify other causes of similar symptoms. Please check the table below for the purpose of each process.
Test ItemWhy It Is NecessaryPerspective at Baekrokdam
Blood TestTo identify other causes such as thyroid disease, inflammatory or autoimmune diseasesWe track the progression of symptoms by recording pain patterns along with fatigue and sleep
Optional TestsAdditional blood tests or imaging tests may be added based on medical history and physical examinationWe evaluate how well the purpose and results of the tests explain your current symptoms
Physical ExaminationTo check the affected pain areas and joint conditionWe observe fear of movement and hypersensitivity to pain
Symptom RecordingTo understand the location and intensity of pain, fatigue, and sleep qualityWe establish benchmarks for recovering daily functioning

What the clinic checks first

If test results are normal, does it mean there is no problem?

Fibromyalgia may not show specific findings in tests. A normal result means there were no clear abnormalities in the items tested; it does not mean that there is no pain or that all other diseases have been ruled out.

More detail

Why Tests Are Necessary

Cannot be confirmed by a single test

Fibromyalgia cannot be diagnosed with one specific test. It is confirmed through a physical examination and by evaluating symptoms and daily functioning.

Excluding other conditions

The same set of blood or imaging tests is not performed for everyone. Based on medical history and examination, tests are selected to check for causes that produce similar symptoms, such as thyroid disease or inflammatory and autoimmune diseases.

Identifying symptom patterns

Whether pain, fatigue, and sleep problems appear together, and how they change according to activity, provide clues for the diagnosis.

Understanding Test Results

Meaning of normal results

A normal result means that no clear abnormalities were found in the items tested. It does not mean that all other diseases have been ruled out at once, nor does it mean that pain and fatigue are absent.

When abnormal findings are present

If there are abnormalities in thyroid or inflammation levels, further evaluation is conducted to see if those results are related to the symptoms. A clinical judgment is made on whether fibromyalgia exists alongside another condition.

Interpretation alongside symptoms

Judgment is not based on test results alone; the location and intensity of pain, fatigue, and sleep status are comprehensively evaluated.

Determining the Next Steps for Care

Recording symptoms

Recording the times when pain is most severe, changes after activity, and the quality of sleep can be helpful for your medical evaluation.

Consultation with relevant departments

Depending on the pattern of symptoms and test results, please receive an evaluation for other causes and the possibility of fibromyalgia at a relevant department, such as Rheumatology.

Lifestyle management

Observe changes in symptoms while adjusting sleep habits and engaging in low-intensity aerobic exercise.

Focus Points at Baekrokdam

Pain Hypersensitivity Type

In cases of widespread tenderness and significant fear of movement, the focus is on restoring the pain threshold.

Sleep Recovery Type

In cases of severe fatigue and not feeling refreshed after sleep, sleep quality and activity patterns are coordinated together.

Multi-organ Type

When brain fog and intestinal symptoms overlap with autonomic nervous system issues, we examine the overall flow of the symptoms together.

At Baekrokdam, we do not simplify fibromyalgia to a single diagnosis or a list of restrictions

Fibromyalgia Testing: Examining what to verify regarding your symptoms

You may have been searching for answers because 'you don't feel refreshed even after waking up in the morning, and whole-body pain persists.' If these changes occurred suddenly, we first verify the necessary tests and medical evaluations. If the symptoms are long-term or recurring, we look beyond a single test value to examine the reason for the test, your symptoms, and previous treatments to decide together what to manage or treat first.
01 · Please tell us first

The most difficult moments and when it first started

Please bring the reason you were recommended for testing, results of tests already performed, changes in symptoms, and a list of current medications.

02 · Things to verify first

Whether symptoms appeared suddenly, or are long-term or recurring

We first distinguish between similar states and warning signs, such as the 'pain-hypersensitive type' (significant whole-body tenderness and fear of movement), the 'sleep-recovery type' (persistent fatigue and lack of refreshment after sleep), and the 'multi-organ type' (overlapping brain fog, gastrointestinal symptoms, and autonomic nerve issues). During the medical evaluation, we follow this sequence: differentiate inflammatory, endocrine, or neurological diseases $\rightarrow$ coordinate sleep, medication, and low-intensity activity $\rightarrow$ recover Qi, Blood, Phlegm-Fluid (Dam-eum), and pain thresholds to expand your range of daily activities. New joint swelling, fever, progressive muscle weakness, sensory abnormalities, weight loss, or thoughts of self-harm should be evaluated immediately and not dismissed as fibromyalgia. For long-term or recurring cases, we examine the diagnosis, response to previous treatments, and the impact on your daily life.

03 · Baekrokdam Consultation

Reviewing previous treatments and remaining discomforts together

If discomfort persists after acute risks are resolved, if there is an insufficient response to existing Western medical treatments, or if options are limited due to side effects, contraindications, or the burden of medication, we review the frequency of symptoms, sleep, activity levels, and treatment history. At this stage, we explain the context and limitations of considering Korean herbal medicine as a concurrent or alternative option; we do not promise specific effects or recommend the arbitrary discontinuation of existing medications.

If it is still unclear what to change in your specific case after reading about fibromyalgia, please come for a consultation with your test results, the reason for the tests, recent symptom changes, previous diagnoses, and current medications. Following the consultation, we will together monitor changes in pain location and intensity, refreshment and fatigue after sleep, possible concentration time, and worsening after activity.

Frequently asked questions

If my test results are normal, does that mean I don't have fibromyalgia?

No. A normal result means there were no clear abnormalities in the items tested; it does not mean that there is no pain or that all other diseases have been ruled out. Symptom patterns and physical examination findings must be evaluated together.

What tests should I undergo?

First, we conduct a detailed review of your symptoms and medical history, followed by a physical examination. Based on those results, we select tests to investigate other causes of similar symptoms, such as thyroid function or inflammation levels; the same set of tests is not performed for everyone.

How long does it take to get the test results?

It varies depending on the type of test, and your attending physician will explain the results. Results must be interpreted in conjunction with your symptoms.
