This is a question I hear very often in the consultation room. It is quite startling when the numbers on the scale suddenly spike. Especially when you are on a diet, experiencing this can make you feel like you've done something wrong and dampen your motivation. However, to give you the conclusion first: that 2kg is mostly water, not fat.

The Reality of Weight Gain: Fat or Water?

Weight gain before menstruation is not simply "fat," but rather the result of edema (swelling) caused by hormonal changes. After ovulation, as progesterone levels rise, the body tends to retain more water. If sodium excretion is not smooth during this time, water leaks out of the blood vessels and accumulates between tissues.

Phlegm-Fluid (痰飲) refers to a state where body fluids do not circulate properly and remain stagnant and viscous. Those in a state of Spleen Deficiency (脾虛)—who usually have weak digestion or low energy—lack the strength to push and excrete fluids. Consequently, even a small stimulus like hormonal changes before a period causes the body to swell easily, which manifests as a higher number on the scale.

I sometimes experience my body feeling heavy when I'm not in top condition. Through my own trials and errors, I've realized that this isn't a problem solved simply by not drinking water. Rather, it is much more efficient to revitalize the function of the Spleen and Stomach (脾胃), which act as the 'pump' for water metabolism.

Will Leaving Edema Untreated Turn It Into Real Fat?

This is exactly what many people worry about: "If the swelling continues, won't it eventually become fat?"

Strictly speaking, edema itself does not turn into fat. However, the problem is decreased circulation. When the body is swollen, blood circulation and lymphatic flow slow down, leading to a decrease in basal metabolic rate and the accumulation of waste products. Furthermore, when the body feels heavy due to edema, physical activity decreases, and a psychological mechanism kicks in where one might give up on their diet, thinking, "I'm swollen anyway."

In the end, rather than the edema itself, the low-efficiency state created by the edema becomes the enemy of dieting. Therefore, instead of viewing premenstrual weight gain as a period that must simply be 'endured,' it is better to focus on how to efficiently drain the stagnant water.

Criteria for Effective Water Management

So, how should you respond? Starving yourself blindly or obsessively doing only aerobic exercise can actually exhaust your body further. Try approaching it based on the following criteria:

First, check your sodium intake. Before a period, you crave not only sweets but also salty foods, and sodium has a strong property of holding onto water. Even eating just a little less salt than usual can significantly reduce the range of edema.

Second, look at the circulation in your lower body. If your calves are particularly swollen and you get frequent cramps at night, there is a high possibility that Blood Stasis (瘀血) is mixed with simple edema. In this case, light stretching and warm foot baths are very helpful.

Third, check your digestive state. If you swell while feeling bloated and gassy, your Phlegm-Fluid (痰飲) is severe. At this time, rather than excessive exercise, priority should be given to drinking warm tea that aids digestion and getting plenty of rest.
