Pounding Headache at 3 PM: The Culprits Are 'Stiff Shoulders' and a 'Starving Brain'

"Every day, without fail, around 3 PM, my head feels like it's being squeezed, as if I'm wearing a tight helmet, and it pounds. Painkillers offer temporary relief, but it's the same story the next day."

This sensation is familiar to any office worker in Korea. While labeled as `tension headache symptoms`, what is the true identity of this unwelcome guest that arrives as regularly as payday? It's too regular to be simple fatigue, and relying solely on painkillers feels unsatisfactory.

There are two causes. Specifically, 'structural issues' and 'energy issues'. First, structural issues are represented by 'over-tension', often manifesting as `shoulder stiffness headaches`.

[CASE]

During a critical project deadline, let's look at the case of Mr. C, a developer in his 30s who sat in front of a monitor for over 10 hours a day. His `pounding headache` symptoms intensified on days when his neck and shoulders felt stiff as a rock.

Our scalp is actually made up of thin muscles and fascia (scalp fascia), which are connected to the neck and shoulder muscles like a vast network. When neck and shoulder muscles become chronically tense due to poor posture or stress, this tension is directly transmitted to the head, causing pain that feels like the entire scalp is being constricted. This is similar to 'how the entire garment twists if you pull on the neck of a tight sweater'.

The second cause is 'brain energy depletion'.

"Strangely, when I skip lunch or grab a quick meal, my headaches seem to get worse."

This is a crucial clue indicating `brain fatigue`. The brain, though only accounting for 2% of our body weight, is an 'energy glutton' that consumes 20% of our total energy. The brain's sole energy source is glucose.

Excessive concentration and stress rapidly increase the brain's glucose consumption. If you skip meals or quickly fill up on high-glycemic index foods, blood sugar levels rollercoaster, interrupting a stable energy supply. Around 3-4 PM is precisely when this energy depletion peaks. An energy-depleted brain becomes hypersensitive even to minor pain signals, amplifying even slight muscle tension into severe headaches.

Intensive work throughout the morning → Increased brain energy consumption → Inadequate lunch → Afternoon blood sugar drop begins → Brain energy depletion → Increased pain sensitivity → Combined with pre-existing neck/shoulder tension → Pounding headache erupts.

In traditional Korean medicine, this condition is viewed as an overlap of 'Qi-Blood Deficiency (氣血虛)' and 'Liver Qi Stagnation (肝氣鬱結)'. It suggests that when the head is not properly nourished due to a lack of energy (Qi) and nutrients (Blood), stress further obstructs the circulation of vital energy (Liver Qi), leading to pain.

[Key to Breaking the Vicious Cycle]

So, how can we break this vicious cycle? Painkillers merely switch off the pain temporarily and cannot be a fundamental solution. Regular stretching for `fascial release` to loosen 'stiff shoulders', and soothing the 'starving brain' with `headache-friendly foods` like whole grains or protein-rich meals that slowly raise blood sugar. Practicing these two together is the true key to breaking the persistent headache cycle.
