Geriatric depression examination: evaluating physical, pharmacological, cognitive, and daily functions alongside questionnaires

There is no single blood test or cognitive test that can definitively diagnose depression in seniors. During a medical evaluation, we conduct interviews regarding emotional numbness, interest, hope, sleep, appetite, pain, memory, judgment, and the ability to manage meals, hygiene, medication, finances, and outings. We also review changes observed by family members and all prescribed medications, over-the-counter drugs, Korean herbal medicine, supplements, and alcohol consumption. Screening questionnaires and cognitive tests are tools to facilitate conversation; they do not confirm depression or dementia based on scores alone. If clues are found during the medical history and physical examination, blood tests may be used to rule out physical or drug-related causes. Sudden confusion, paralysis, or changes in consciousness require an emergency evaluation before scheduling a routine test.

Guide to Depression Screening for Seniors

Geriatric Depression: Illustration for this condition

Key points

  1. 1Screening questionnaires and cognitive tests are not definitive diagnoses; the core of the process is the interview and the evaluation of function and safety.
  2. 2Depression and dementia are not distinguished by a single score; instead, we look at the speed of onset, progression, daily functioning, and potential physical or drug-related causes.
  3. 3Suicide plans, acute confusion, paralysis, or changes in consciousness require immediate protection and emergency evaluation.

Testing Workflow

Connecting screening, interview, functioning, and physical/cognitive differentiation

Rather than relying on a single score, we interpret the speed of onset, daily functioning, and acute risks together.

What the clinic checks first

If the cognitive test is normal, does that mean it's not depression?

No. A cognitive test is just one tool to assess memory and thinking functions. Depression is determined by synthesizing interviews, functioning, safety, and the differential diagnosis of physical and drug-related causes.

More detail

Screening tests initiate conversation and track changes

Assessing emotional numbness, interest, and thoughts of death

Depression is not ruled out simply because sadness is absent; we directly check for pleasure, hope, and suicidal ideation.

Diagnosis is not based on scores alone

Scores are interpreted alongside hearing, vision, language, education, physical discomfort, family observations, and actual daily functioning.

Comparing functioning before and after treatment

Along with scores, we monitor whether sleep, appetite, outings, and medication management are actually improving.

Reviewing physical illnesses, pain, and polypharmacy

Determining the direction of differential diagnosis through history and examination

We check for clues related to pain, neurological issues, or thyroid dysfunction, as well as nutrition, dehydration, and recent infections or hospitalizations.

Reviewing all substances in a single list

We compare prescribed medications, over-the-counter drugs, Korean herbal medicine, supplements, and alcohol, noting start dates, dosage changes, and the timing of symptoms.

Selective testing only for those who need it

Blood tests and other diagnostics are selected based on suspected causes; imaging and tests are not performed uniformly for everyone.

Directly evaluating cognition, function, and acute safety

Combining memory tests with daily functioning

We synthesize cognitive tests with the speed of onset, mood changes, and the ability to manage medication, money, appointments, cooking, and hygiene.

Ruling out delirium and stroke first

Sudden fluctuations in attention or consciousness, fever, paralysis, or changes in speech require immediate emergency evaluation.

Specifically asking about suicide risk

We check for plans, means, and attempts, as well as behaviors such as hoarding medication, settling assets, or the amount of time spent alone.

Baekrokdam's perspective on geriatric depression screening

We prioritize acute safety, polypharmacy, cognition, and self-care abilities over simple scores

Baekrokdam first screens for sudden suicide risk, delirium, and neurological symptoms. After maintaining the necessary cognitive and physical evaluations provided by primary care and geriatric psychiatry, we narrow the scope of herbal medicine treatment consultations only when chronic sleep, digestion, pain, or vitality issues impair overall functioning.
01 · Acute Safety

We first check for suicide risk, confusion, and paralysis

We evaluate plans and means, changes in attention, consciousness, or neurological status, and the ability to manage meals and medication.

02 · Comprehensive Evaluation

We examine mood, cognition, physical health, and medications together

We integrate all substances taken, pain and medical conditions, and the timeline of memory, judgment, and daily life management.

03 · Follow-up

Compare test scores with actual function

Record the management of sleep, diet, outings, medication, finances, and appointments, as well as treatment effects and side effects.

Please prepare information regarding the onset of symptoms, changes in sleep, diet, weight, pain, medication, finances, appointments, and hygiene, as well as all prescription drugs, over-the-counter drugs, Korean herbal medicine, and supplements.

Frequently asked questions

Is geriatric depression confirmed through blood tests?

There is no blood test that can definitively diagnose depression. Blood tests are selected based on medical history and physical examination to rule out suspected physical causes.

Do I need to undergo both depression and dementia screenings?

If there have been changes in memory, judgment, and daily functioning, both a depression interview and a cognitive assessment are considered. The scope is determined based on the individual's situation.

How should I prepare the medications being taken?

Please bring a single list including prescriptions and medication envelopes from all hospitals, as well as over-the-counter drugs, Korean herbal medicine, supplements, and alcohol use.

Can a family member explain the symptoms instead?

While family observations are important, we also listen to the senior's own experiences directly. We compare both sets of information to examine the timeline and functioning.
